TERMS & CONDITIONS for HIPAA Express® workshop
The following constitute the terms and conditions to which registrant agree when booking any Carosh Media and Marketing llc, dba Carosh Compliance Solutions llc (“Carosh”) service (Terms and Conditions), which may be reviewed, revised and/or periodically updated.
1. REGISTRATION WITH CAROSH
When registering for a HIPAA Express® Workshop you will be asked to provide us with certain information about yourself and your organization. In doing this, you agree to:
- Provide true, accurate, current, and complete information about yourself and your organization as prompted by the registration form.
- Maintain and promptly update the organization and contact data we hold within HIPAA Suite®, as necessary.
2. BOOKING AN IN-PERSON OR VIRTUAL HIPAA EXPRESS® WORKSHOP
2.1 Workshop(s) can be booked via the registration process available on Carosh’s website, carosh.com/hipaaexpress.
2.2 When you register and pay the non-refundable deposit of $250.00, for an in-person or virtual HIPAA Express® Workshop, your submission represents your intention of purchasing the full HIPAA Express® Workshop. Upon registration for the Day 1 Session and Day 2 Session, you will receive automated email(s) regarding your selections.
2.3 Carosh will accept your purchase intention by reserving your space at the sessions that you have registered for. Reminder emails will be sent prior to the session(s) you have registered for.
3. PRICES, DISCOUNTS AND SALES TAXES
3.1 Notice of new pricing will be given in advance, but Carosh reserves the right to change prices listed without notice.
3.2 From time-to-time discounts may be available on Carosh services. The discount and its timing are at the sole discretion of Carosh.
3.3 If you select a special rate or discount on the registration form, you may be required to provide evidence of your qualification for the special rate or discount.
3.4 The in-person session fees include: tuition on the day(s), all relevant course materials, and may include refreshments, breakfast, and lunch.
4. PAYMENT
4.1 The non-refundable deposit of $250.00 must be paid at the point of registration via Carosh’s secure online payment process. If you have an applicable discount code this must be entered at the time of registration, as the discount cannot be applied retrospectively. Registrants have the option to pay in full during the registration process. If they choose to pay only the non-refundable deposit, the remainder due must be paid prior to attending any sessions of the HIPAA Express® Workshop, whether in-person or virtual.
4.2 All major credit and debit cards are accepted. A receipt will be sent to you by email, from Carosh, confirming payment. Carosh will receive the information needed to verify and authorize your payment card and to process your order and is under strict legal and contractual obligations not to disclose this information to third parties. Please note that if you do not provide accurate details (including type of card and number) or if your credit card company does not authorize payment, your application will be deemed void. Carosh will not accept any liability for costs incurred as a result of applications deemed void in this manner.
4.3 Sales taxes, as required, are charged at the applicable rate depending on the product and/or customer.
4.4 Installment agreements must be discussed with Carosh and accepted by Carosh prior to the attendance of any HIPAA Express® Workshop sessions.
5. PREVAILING TERMS AND CONDITIONS
Please note that this and all other Carosh pre-contractual documentation shall not constitute an offer. All services provided by Carosh are based on these Terms and Conditions and if you wish to engage any Carosh services we shall only provide them if you accept the Terms and Conditions. If you offer to engage Carosh on your own terms and conditions then we shall only accept your offer subject to the Terms and Conditions, which shall, if we provide the services you offer to engage, prevail over any other terms and conditions.
6. CAROSH in-person or virtual sessions
6.1 Subsequent to the first session break, registrant will be provided with the opportunity to terminate their participation in the workshop. A written notice of termination shall be required. Included in the notice of termination will be a statement for the reason for termination. In the event registrant terminates their participation, all fees, excluding the non-refundable portion of the fees will be returned within 24 hours. Access to all services (workshop and HIPAA Suite®) will be immediately terminated.
6.2 While Carosh makes every effort to keep changes to the site to a minimum, we reserve the right to make any amendments to the terms and conditions at any time to reflect changes affecting our business, including but not limited to, changes in technology, payment methods and legal and regulatory requirements. By continuing to use this site you agree to accept any changes made to the terms and conditions.
7. OWNERSHIP AND INTELLECTUAL PROPERTY RIGHTS
All materials, whether on the website or at a HIPAA Express® Workshop session, are owned by Carosh. All intellectual property rights in all materials available from the workshop, including the design, graphics and text of all printed materials and the audio of all webinars and recordings, are owned by Carosh. When you are given access to HIPAA Suite®, you are granted a non-exclusive, non-transferable, revocable license to use the site. No content may be copied, reproduced, uploaded, posted, displayed, or linked to in any way, in whole or in part, without Carosh’s prior permission. Any such use is strictly prohibited and will constitute an infringement of Carosh’s intellectual property rights.
8. SPECIAL REQUIREMENTS AT IN-PERSON HIPAA EXPRESS® WORKSHOP SESSIONS
Registrant will be required to provide their own portable computer (laptop) with a working wireless access point.
Carosh will make every effort to accommodate special requirements that have been notified in advance on the registration form.
9. HEALTH AND SAFETY AT IN-PERSON HIPAA EXPRESS® WORKSHOP SESSIONS
You must comply with all health and safety rules and regulations and any other reasonable security requirements that apply at the premises at which the sessions are provided.
10. INVITATION LETTERS
Should a participant require an invitation letter from Carosh, we are able to provide this as long as full payment of the workshop has been received. Carosh is not able to act on behalf of the participant and is not responsible for any costs incurred by failure to obtain a full visa.
11. BEHAVIOR
Carosh reserves the right to remove any participant from a session whose behavior is deemed inappropriate by Carosh or its trainers or representatives. In these circumstances, Carosh will neither refund any fees nor reimburse any other costs.
12. TRAINERS or representatives
Carosh shall provide such trainers or representatives to present the session as it, in its sole discretion, deems fit and Carosh shall be entitled, at any time, to substitute any trainer or representative with any other person who, in Carosh’s sole discretion, it deems suitably qualified.
13. LIABILITY
Carosh does not accept responsibility for anyone acting as a result of information in, or views expressed during, sessions or on course materials. Opinions expressed are those of individual trainer or representative and not necessarily those of Carosh. Participants should take professional advice when dealing with specific situations.
14. TECHNOLOGY
Carosh does not warrant that the provision of any content online will always be available or be uninterrupted, timely or error free, that defects will be corrected or that such content is secure or free from bugs, viruses, errors and omissions.
15. Virtual JOINING INSTRUCTIONS
Virtual joining instructions including an outline of the day and venue details will be available prior to the session start date via email. Carosh will not be held responsible for non-receipt of virtual joining instructions and refunds will not be issued under such circumstances. It is the responsibility of the attendee to ensure they have received all necessary information prior to the session they have registered for.
16. CANCELLATION
16.1 Cancellation of a registered session. If you need to cancel your place in a session, you must notify Carosh via email. If Carosh is notified more than 14 days before the session start date, there will be no charge. Any substitutions made less than 14 days before the session start date will incur a $150 administration fee.
16.2 Substituting your registered session place. If you need to substitute your place in a session, you must notify Carosh via email. If Carosh is notified more than 14 days before the session start date, there will be no charge. Any substitutions made less than 14 days before the session start date will incur a $150 administration fee.
16.3 Transferring your place. If you can no longer attend the session you are entitled to transfer your registration to another session that has availability. The transfer is free of charge if you notify Carosh via email more than 14 days before the session start date. Any transfers made less than 14 days before the session start date will incur a $150 administration fee.
16.4 Changes and/or Cancellation by Carosh. Where circumstances dictate, Carosh reserves the right to alter published programs, trainers, fees, dates, or venues without prior notice. In extreme circumstances, Carosh may be forced to cancel a session on 14 days’ notice. In that case the registrant will have the option of attending another available session. No compensation will be paid for any additional costs incurred (for example travel, accommodation etc.).
16.5 Non-attendance. If you fail to attend the session you are registered for without giving prior notice to Carosh, we will refund the course fees or offer a transfer, per the terms outlined in paragraphs 16.1 – 16.3 above.
